
There are several ways to access the Diabetes Education Centre:
1. Physician’s Referral:
Your doctor will complete a referral letter and fax it to the Diabetes Education Centre. Once we receive the referral you will be contacted by phone to arrange for an appointment or by mail with a scheduled appointment. A mail-out package will be sent to you, or you can download our Self Assessment Questionnaire here.

2. Self Referral:
You may refer yourself to the Diabetes Education Centre. We are a full time program, however our receptionist is available part-time. She is in the office Mondays, Tuesdays, and Thursdays 8:45-3:00pm; and Wednesdays and Fridays 8:45-12:00pm. If you get the answering machine please leave your name and phone number and she will get back to you as soon as possible. At that time you will be given an appointment time and mailed out an information package.
3. Caregiver or other health care provider referral:
Your caregiver or health care provider can telephone our office to make the referral.

They will need your full name and telephone number. Then we will contact you directly to arrange the appointment. At that time a mail out package will be sent to you. If you are not able to physically come to the office your family member or other caregiver can schedule an appointment to attend on your behalf.
